Indiana Health Centers to Open Whole Person Connection Hub in Seymour, Indiana
Whole Person Connection Hub will provide full range of healthcare services for the Seymour community and shared space for community partners
SEYMOUR – Indiana Health Centers (IHC) will expand its existing Seymour, Indiana, location into a full-service Whole Person Connection Hub. In addition to primary medical care, the Hub will provide behavioral health services, collaborative space for community partners and a pharmacy.
Indiana Health Centers will be hosting a grand opening of their Whole Person Connection Hub in Seymour, Indiana on Thursday, August 6 at 10 a.m. at our IHC Jackson County location at 120 St. Louis Avenue, Seymour, IN.

We’re grateful to our community partners and our grant funder for helping us create a space where collaboration can truly make a difference in the lives of the people we serve.”
Those partners include the Community Foundation of Jackson County, which was awarded a $3.443 million GIFT VIII Implementation Grant from Indianapolis-based Lilly Endowment Inc. to help fund the project along with the creation of three substance use disorder recovery homes through Centerstone.
“The Foundation staff and Board of Directors are excited to see this project coming to fruition, enhancing and creating new services to help our families, friends and neighbors find the physical and mental health services needed here in Jackson County, Foundation President & CEO Dan Davis said.
A portion of the grant — $2.4 million — funded renovations, furnishings and equipment of the Whole Person Connection Hub. The Foundation also established the Community Foundation Mental Health Fund through the GIFT VIII Implementation Grant to provide annual operating support to the mental health services of the Hub. This fund will pay sustainability grants each spring in perpetuity.
The Foundation also worked to bring other funders into the project, including the use of opioid settlement dollars from the City of Seymour and the Jackson County Board of Commissioners and County Council; the Schneck Foundation; and the Central Pharmaceuticals/Virginia B. Toulman Foundation Fund and the Jackson County Community Endowment, two funds administered by the Foundation.
The Whole Person Connection Hub aims to be a one-stop shop for IHC patients. The hub will include:
- Expanded Behavioral Health: this includes a new space for two behavioral health providers, a psychiatric mental health nurse practitioner, and working space for medical assistants including a lab.
- Collaborative Space for Community Partners: the Whole Person Connection Hub will include office space for community partners like Catholic Charities, 180 RCO, Su Casa, Schneck, Centerstone, and Anchor House. The space will assist in promoting collaboration in-house for additional resources and services with organizations that are in the community.
- Community Kitchen: The hub will feature a kitchen with full kitchen appliances and a large space to host classes. IHC will use the space for cooking classes and nutrition guidance.
- Pharmacy: IHC will offer an in-house pharmacy to provide healthcare and pharmacy in one convenient location.
Many IHC community partners in Seymour have already indicated they will be using the space.
“Schneck will be using this space as a continuation of patient care,” said Meghan Warren, Director of Community Health and Equity at Schneck Medical Center. “In addition, our Community Health Workers will find this space extremely helpful as it will provide them a space to meet with individuals in a private, safe space supporting their recovery journey.”

We look forward to stronger care coordination and warm hand-off referrals, all in a welcoming environment that is familiar and trusted” said Ashley Caceres, Executive Director of Su Casa Indiana. “We are so excited to be in this new space.”
“We would love to use the drop-in space at IHC during times we are meeting clients, especially during the summer months when school is out of session and clinicians need a place to conduct appointments with youth clients,” said Brittany Drawbaugh, Manager of Adult and Family, Indiana at Centerstone. “We are also very much looking forward to using the community kitchen. We are grateful to have IHC as a community partner to help expand the availability of behavioral health care.”
